What forms do I need?

Please take a moment to print and complete the following forms prior to your appointment.

New patients should complete a Patient Information Form (*).

Please complete and sign the appropriate consent form prior to a surgical procedure:

When should I arrive?

For surgeries, please arrive between 8:00 and 9:30am.

For regular office calls, please arrive a few minutes before your appointment. If you are a new client, please arrive about 10 minutes prior to your appointment.

How should I prepare my animal?

Do not give your pet any food or water after midnight the night before any surgical procedure.

What else should I know?

How can I pay?

We offer numerous payment options, including: cash, checks, credit cards (Visa, Mastercard, Discover, American Express), and money orders. As a rule, payment is expected at time of service.

Care Credit Instant financing is available through Care Credit, a special credit card just for your pets' health needs. This flexible healthcare payment plan allows you to pay for your pet's treatment over time, without any up-front payment required. More information can be found on their website and you can apply for credit through our office.

There are times when unexpected emergencies arise and unexpected procedures are needed. Accordingly, we also offer some limited in-hospital financing for clients in good standing for emergency/sick animals only. Our payment plan requires at least half of your balance be paid at the time of service, with the remainder of the balance to be paid with a monthly payment plan over the following 3 months. A Credit Application (*) must be completed for approval.
